Psicoterapia e carisma

L’io si nasconde anche nei meandri dell’antropoanalisi

Piero Balestro (1938-1994), teologo e psicoterapeuta, è stato uno studioso dell’Antropoanalisi di Ludwig Binswanger i cui principi egli ha applicato alla pratica psicoterapeutica. A dieci anni dalla morte, si possono ripercorrere le tappe del suo essere terapeuta attraverso valori fondanti dell’Ordine Fatebenefratelli. Antropoanalisi significa porre al centro dell’attenzione l’uomo nella sua peculiarità ed unicità. L’idea di centralità che io ho ritrovato nella pratica ospedaliera dei Fatebenefratelli e negli scritti di fra Pierluigi Marchesi. “Spesso si dimentica che la componente più preziosa della società non è lo Stato o la Chiesa o l’Istituto, non è la legge, non è l’organizzazione politica o quella del lavoro, ma è la personalità, cioè la persona nella sua unicità ed irripetibilità.” (Marchesi, Per umanizzare l’ospedale, Centro Stampa Fatebenefratelli,  1983).
“Essere uomini implica individualità del pensare, responsabilità personale nelle scelte, originalità contro pregiudizi o luoghi comuni, solitudine dell’adulto, creatività ed inventiva” (Balestro, La tristezza inutile, 1989). L’uomo è secondo Balestro (nella foto) un “evento vitale”: scopo della vita è vivere ed essere felici: lo dice la Bibbia “Non privarti d’un giorno felice” (Siracide 14,14), lo dicono la tradizione popolare, la letteratura, la natura stessa dell’uomo. Mettere la persona al centro significa, prima di tutto, riconoscere se stesso come persona con i propri bisogni e le proprie sofferenze. Marchesi scrive: “uno dei più ardui compiti che noi incontriamo per cercare di umanizzarci per poter umanizzare l’ospedale è quello di riaccendere il ‘contatto con noi stessi’, con il nostro corpo, la nostra anima, la nostra psiche… Se vogliamo costruire nell’ospedale il tempio, dobbiamo ricordarci che questo tempio è abitato da uomini e da donne, da figli di Dio; chi rifiuta parti di sé, chi non porta se stesso, oltre alla propria competenza, non è in linea con la missione, in quanto non offre all’altro lo spazio affettivo necessario per una relazione integrale d’aiuto” (Marchesi, Alle donne consacrate nel mondo della sanità, Ed Fatebenefratelli, Milano, 1977).
Nel riconoscersi come uomo è però essenziale riconoscere anche l’incontro con l’altro. In Parlare l’amore Balestro dice: “Prima di ogni incontro singolo, l’altro è il bisogno insito in me stesso di completarmi con …, la direzione di quella mia intima necessità per sopravvivere non solo fisicamente, la risposta del mio non bastare solo a me. L’altro così individuato indica che nessuno, per sua intrinseca necessità può viversi senza relazionarsi a … quindi la relazione esiste innanzitutto come struttura dell’essere (Balestro, Parlare l’amore, 1993)
Di Piero Balestro mi ha sempre colpito la capacità di accordare e di aiutare gli altri ad accordare la creatività con il realismo, dove la sua ironia, che sapeva talvolta un po’ di trasgressione, si abbinava al profondo rispetto dell’altro in un atteggiamento di saggezza pratica, serena, in tutti i suoi rapporti personali, fossero essi professionali o amicali. Rispettare dell’altro significa riconoscere che l’altro non è mai staccato da me, non è mai fine a se stesso. Essere uomini significa incontrare: essere un noi nell’autenticità. La persona che si unisce ad un’altra nel noi non si perde nella singolarità del mio o del tuo. La singolarità dell’uomo non si annulla, ma si rafforza.
Nell’incontro autentico l’altro non è né un mezzo né un fine. È, esiste come un struttura fondamentale del nostro essere, concorre a definire la nostra stessa realtà di uomini. Succede anzi che la presenza viene esaltata proprio nel momento della donazione di sé. Nella Carta di Identità dell’Ordine Fatebenefratelli si legge: “L’Ordine Ospedaliero deve distinguersi per la disponibilità e il servizio ad attuare, nella misura del possibile, la realizzazione pratica ed effettiva dei principi di  integrazione, normalizzazione e personalizzazione”(4,2.2) che si traduce in ciò che Marchesi chiamava il dovere di “Ospitare l’umanità, ovvero ospitalità per l’umanizzazione … come obiettivo strategico”.
Nell’ottica dell’accogliere l’uomo sofferente, si poneva anche il lavoro di Piero Balestro come psicoterapeuta; egli infatti sottolineava la necessità del coinvolgimento emotivo nel rapporto. Come il suo maestro Binswanger che chiamava lo psicoterapeuta Mitspieler (colui che gioca con … che si mette in gioco), Balestro dice che lo psicoterapeuta deve avere “un atteggiamento di commozione, di simpatia, di solidarietà, di partecipe misericordia verso la giornata del vivente, così patetica, intrisa come è di desiderio e illusione, di consapevolezza e di oscurità. Avendo conosciuto nella sua carne la contraddizione, la debolezza, il vano opporsi alla necessità che si trascina ad essere senza che te ne accorga, lo psicoterapeuta non si stupisce, né si sdegna nel constatarla attorno a sé: tutto è comprensibile, anche se migliorabile, tutto è umano anche se dolorante” (Balestro, La tristezza inutile, 1989). Questo mi sembra il segreto di Piero Balestro come psicoterapeuta: come direbbe Binswanger, “essere con l’altro ed essere per l’altro in un modo veramente umano”.
La parola chiave che sembra accumunare due mondi, quello della psicoterapia di stampo antropoanalitico, binswangeriano e quello della pratica ospedaliera dei Fatebenefratelli è a mio parere la condivisione. Condividere significa “partecipare”. Il rapporto tra lo psicoterapeuta e il suo paziente o tra il personale sanitario e il paziente ricoverato porta inevitabilmente alla partecipazione ad un medesimo destino, anche se per un periodo di tempo limitato. Ogni rapporto terapeutico ha la sua radice nella sofferenza: solo chi l’ha vissuta in prima persona può essere in grado di riconoscerla e condividerla. “Il mondo della condivisione abbraccia il dolente come se stesso: in lui condivide un destino che non è solo suo; in lui soffre l’eterna vicenda umana, che, come mette a vivere, ti toglie; come ti fa sperare, ti disillude; ti spinge ad osare mentre uotidianamente ti falcidia. Quanto succede a lui è quindi naturale; come naturale è occuparsi di lui, proprio come durante un cataclisma. Il dolore quotidiano è più di un agente eccezionale: è la regola; una regola omniavvolgente.” (Piero Balestro, La tristezza inutile, 1989).
Fra Marco Fabello direttore generale del Presidio ospedaliero B V Consolata di San Maurizio Canavese, vicino a Torino, sostiene: “Passando oltre l’uomo sofferente e ferito, perdiamo l’occasione di essere strumenti sananti. C’è una sofferenza interiore che spesso non è vista ma è più dolorosa. Per riuscire a sconfiggere la sofferenza, bisogna che questa sia prima
riconosciuta. accolta, incontrata, condivisa: in un certo senso, per sconfiggerla bisogna prima convivere con la sofferenza stessa” (2002). Ogni operatore sanitario si trova quotidianamente ad affrontare il problema del dolore inevitabile che si porta dietro sentimenti di impotenza, paura, ribellione. Di fronte a tal genere di dolore (la malattia, la morte) Balestro individua due atteggiamenti tipici dell’uomo (e di conseguenza anche dell’operatore sanitario): quello di chi si vive “da eterno”, si prodiga in consigli –il più delle volte inutili– preda di un narcisismo irritante che lo vuole comunque protagonista della scena e quello di chi, nel dolore, parla “come stando zitto”: è il silenzio quasi contemplativo di chi abbraccia, accoglie e comprende, è la condivisione vera che si esprime nei gesti più semplici ma, al momento, più  indispensabili. Il tentativo di stabilire un legame umano è l’anima vera del trattamento terapeutico di qualsiasi tipo. La parola che suggella questo tipo di legame è una parola cristiana, ma può essere messa in pratica anche da parte di coloro che cristiani non sono: è la parola Amore.
Per ogni persona impegnata nell’ambito della sanità, la forza terapeutica dell’amore è senz’altro una affermazione impegnativa, ma di efficacia dirompente. “Solo l’amore è in grado di calarsi nel dolore, di capirlo, di piangerlo e al tempo stesso di alleviarlo: infatti solo l’amore è capace di sopravvivere comunque, di farti sentire unico, indispensabile, importante nonostante l’ormai tua insignificanza, il tuo cader nel nulla … (Piero Balestro, La tristezza inutile, 1989). Ancora una volta, in quest’ottica, scopriamo che l’altro ci realizza e ci fa essere. È una realtà con cui si deve, nostro malgrado, fare i conti perché l’uomo come singolo di per sé non esiste. La grandezza di Balestro è stata la sua umanità: era in grado di riconoscere la sofferenza e di non passare oltre, ma di farsene carico e di condividerla con te, sia nelle vesti professionali che in quelle di amico.
